To compare effects of a multidisciplinary approach (MTG) and aerobic interval training (AIT) on cardiovascular risk factors in overweight adolescents.

Sixty-two overweight and obese adolescents from Trøndelag County in Norway, referred to medical treatment at St. Olav's Hospital (Trondheim, Norway), were invited to participate. Fifty-four adolescents (14.0+/-0.3yrs) were randomized to either AIT (4x4-minutes intervals at 90% of maximal heart rate, each interval separated by 3-minutes at 70%, 2/week for 13-weeks) or to MTG (exercise, dietary and psychological advice, 2/month for 12-months). Follow-up testing occurred at three- and twelve-month.

VO2max increased more after AIT vs. MTG, both at three- (11% vs. 0%) and twelve-month (12% vs. -1%) (p<0.01). AIT enhanced endothelial function compared to MTG at both three- (absolute changes 5.1% vs. 3.9%) and twelve-month (absolute changes 6.3% vs. 1.0%) (p<0.01). AIT was favorable compared to MTG in reducing body mass index, fat-percent, mean arterial blood-pressure and increasing peak oxygen pulse. Additionally AIT induced more favorable regulation of blood glucose and insulin compared to MTG.

Novel findings of the present proof-of-concept study was that 3-month of twice weekly high intensity exercise sessions reduced several known cardiovascular risk factors, more than that observed after a multi treatment strategy in obese adolescents, that was initiated as hospital treatment. Twelve-month follow-up confirmed that AIT improved or maintained these risk factors to a better degree than MTG.
